To enroll in the CSCN, please work with your
healthcare professional.
Your healthcare professional will prescribe
NEXPLANON within their electronic medical
record (EMR) system. Less commonly, you may
need to fill out an Enrollment Form.
Once your healthcare professional has
submitted the prescription for NEXPLANON
(either electronically or via an
Enrollment Form), they will walk you
through next steps.
