To enroll in the CSCN, please work with your health care provider (HCP).
You and your HCP are both responsible for reading and filling out the appropriate sections of the Enrollment Form. You will need to sign and date the form once all information has been completely filled out.
Once you and your health care provider complete and submit the CSCN Enrollment Form, your health care provider will walk you through the next steps.
